I would like to share my experience on getting my passport at DFA San Fernando, I hope this could help others since I also look in different blogs for getting my passport but its kinda hard to find a latest one so as for newbie it’s hard to totally depend on it hehe.. anyways I’ll just list down what happened to me haha.
First I secured an appointment online (DFA Site) but it took 3days for me to be able to set an appointment .I’m not quite sure if it’s because of my internet connection or just due to traffic on the site. 3days because there will be a time that there will be no more slot available so I need to start over again and I think there’s also a time limit in filling up the application form because it also happen that my time expires so again I have to start from zero. After filling up successfully DFA sent the copy of the application form into my email and just need to print out the copy in A4 size.
Date of my Appointment at DFA: Nov. 8, 2016 I chose 11am on the time slot but in DFA I believe it doesn’t matter since you will be given a number when you arrive.
For the requirements:
(They say that the older the better when it comes to your documents but if you could bring both the old and the latest one why not bring them all just to prove your identity)

*2 valid ID (Photo copy) – I only have my Pag-ibig Loyalty card and Philhealth ID, but it’s much better to have the UMID from SSS or the new Postal ID. In my case, during verification they ask for my old student ID but It happen that I’m not able to keep one so I have nothing to present. Gladly they accept it. :D
*NSO birth Certificate (Original) – I’m not aware that time that the latest NSO birth certificate has a different logo from the old one but still they accepted the old version as well as the new version (I’m sure of this because my bf brought the old version and he was able to get his passport hehe). The new one has the PSA logo and the old version has NSO.
*T.O.R (Photocopy)
*Voter’s certificate of registration – I secure to have this certificate of registration since I still don’t have my Voter’s ID for 5years as a registered voter hehe and since my 2 available ID doesn’t belong to the list of acceptable ID.
*Brgy. Clearance, Police clearance and NBI Clearance (Photocopy) – serve as my supporting ID/documents
*SSS E-1 form (Photocopy) – My SSS ID will still arrive 3 months from the day of my appointment so I decided to bring my E-1 form as well as the acknowledgement stub for SSS ID application just to prove that I am already a member of SSS I just didn’t have the id and it’s in the list of supporting docs

Actual day of application:
I arrived at the DFA San Fernando (Robinson bus terminal) around 10am, there’s already a long line going to entrance but before falling in line I’ve notice that there’s a person in uniform holding lots of application form so I ask and confirmed if he is a DFA official and if I need to pass my printed application to him so I could have my number. I signed my application form and give it to him. At exactly 10am (Mall hours) we move to the 2nd floor at the DFA office Then upon arriving there the official told us to wait for our names to be called, they will return the application form with number.. They have a photocopy area inside so no worries when you forgot to have your documents photocopied, just need to pay 2pesos per page hehe. There’s a monitor that you will see if it’s your turn already. Verification process is the first step where in they will check the documents, once passed next is the cashier and the last one is biometrics. It only takes an hour for me to finish the whole process , thought it would take whole day but glad to finish early.
For rush: 1,200 PHP it takes 15days (but still expect for delays)
For regular: 950 PHP – I choose these one since I’m not in a hurry hehe. And I got my passport a month after. (Dec. 8, 2016)
They have a delivery option I am not sure how much and how long it will take to receive the passport.
P.S. DFA have a waiver form that you need to sign , its about the possible delay on processing the passport, I was skeptic in signing since it’s like you’re accepting that any delay is just okay with you but still I signed since I’m afraid they will easily deny my application hehe.
